Twin Oaks Care Center has established an outstanding reputation in North-Central Ohio for its excellence in Nursing Care, Rehabilitation Services, and for providing resident services in a homelike setting where residents can enjoy freedom of choice in maintaining their dignity, privacy, and quality of life.
The Rehabilitation Services are outcome based and include psychosocial interventions by a licensed Social Worker. Twin Oaks Care Center is fully certified by Medicare Part A and B, Medicaid, and many Private Insurance reimbursement programs.

The Center's staff members include a Gerontological Certified Nurse, Certified Activities Coordinator, Licensed Nursing Home Administrator, and an experienced team of nursing, housekeeping, dietary and support personnel.
DINING FACILITIES
At the Twin Oaks Care Center, the importance of resident dining programs are recognized as a vital aspect of services that promote an enjoyable living experience for its residents. Residents are encouraged to participate in menu selection by interacting with the Licensed Dietician and the Food Service Supervisor.
In the attractively decorated Dining Room, residents can enjoy delicious meals which are both appetizing and well-balanced. All dietary services are prepared in accordance with physician orders and the specific needs of each resident.
RESIDENTIAL LIVING AREAS
The facility has private and semi-private room accommodations available which offer the essential comforts of home to each resident. All rooms are equipped with electronic nurse call systems, adjustable beds, telephone and television connection outlets, and each room is equipped with a full bath.
PHYSICAL THERAPY
Physical Therapy assists residents with regaining maximum physical independence, by helping to increase strength and joint mobility. Also, PT can improve coordination, endurance,and teach skills for balancing, bed mobility, transfers and ambulation.
OCCUPATIONAL THERAPY
Occupational Therapy assists residents with regaining maximum physical independence, by focusing on upper extremity functions, coordination, and cognitive/perceptual skills. OT also performs "Home Assessments" and develops home therapy programs for after-discharge from the facility, and provides instruction on the use of adaptive equipment.
SPEECH AND LANGUAGE THERAPY
Speech and Language Therapy assists Residents with communicating wants and needs, improving skills to understand and express language, minimize voice and fluency disorders, and increases residents ability to chew, clear their mouth and swallow safely.
