St. Francis Health Care Centre provides wholistic long-term care and rehabilitation
services in a dignified, joyful and compassionate manner in the service of God's people.

Located in Green Springs, Ohio, in the heart of Northwest Ohio, St. Francis Health Care Centre provides compassionate care for the elderly and physically challenged. We are a healthcare ministry under the sponsorship of the Franciscan Sisters of Our Lady of Perpetual Help based in St. Louis, Missouri. As a Catholic facility, we respectfully offer those we serve care, hope, and dignity in a joyful and compassionate manner. Utilizing a holistic approach to health care, we are committed to excellence in teaching those we serve to adapt to their disabilities and to maintain the highest level of independence. We strive to meet the changing needs of the physically challenged and of the elderly by continually assessing the services we provide. Seven integrated medical and physical rehabilitation programs are offered within its clinical units:

* A 36 bed Long Term Acute Care Hospital (LTACH) for acute physical rehabilitation and medically complex patients.
* A 148 bed Nursing Facility (NF) for general physical rehabilitation, subacute medical patients, long-term residents and short stay convalescence.
* A Therapy Center with physical, occupational, speech/hearing, and aquatic therapies for inpatients and outpatients

The Nursing Facility at St. Francis accommodates both long term residents and short stay patients.

The Residential Care Program provides long term nursing care. It has an end-of -life program which includes hospice services.

The Short Stay Convalescence Program is for individuals who are recovering or regaining strength to return to their home. They receive therapy as prescribed. All the programs and activities of the residential program are available to them.

Attending physicians are on call 24 hours and see residents regularly. The nursing home has 24 hour registered nursing coverage, quarterly team conferences with patients and family members, and access to therapy and clinical services of the hospital.

Each resident room has its own private half bathroom. Most rooms are single occupancy. Electrically operated beds, closets, and basic room furniture are provided. Residents are encouraged to individualize their rooms with pictures, entertainment centers, and personal furniture.

A dining room and areas for activities are available on each of the three units of the nursing home. Residents and their visitors are welcome to enjoy the common areas including the lobbies, courtyard, and the park around the pond.

Daily restorative programs encourage residents to remain as independent as possible. Recreational activities use the grounds, greenhouse, multipurpose gym, activities room therapeutic pool, and surrounding communities of Green Springs, Clyde, Tiffin, and Fremont.

Patients meet weekly with their case manager and care givers at team meetings to set goals, review progress, and discuss their care.

The Subacute Medical Program provides patients with skilled nursing care for chronic disease management or prolonged physical recovery. Treatments and therapy are administered as prescribed.

The General Rehabilitation Program provides patients with physical, occupational, and speech therapy as appropriate, along with skilled nursing services. Typical diagnoses are joint replacement and other orthopedic surgery, wound care, post-cardiac surgery, diabetes, mild stroke, arthritis, and general reconditioning.

Education is also incorporated into every patient's care plan. Patients and family members learn about maintaining special diets, administering medications, performing treatments, providing daily care, continuing exercise programs, and using adaptive equipment.
