For over 126 years, Montefiore has cared for Cleveland's aging Jewish and general community. We continue to provide excellent and comprehensive care to individuals as they age and support to families. We offer a full continuum of care including post-hospital, homecare, dementia, long-term nursing home care, assisted living and hospice services.
In 2007, Montefiore celebrated its 125th anniversary and completed a three year expansion and renovation project. This included the refurbishment of all resident rooms, new atrium, offices, Shabbat elevator, gardens, courtyard, children's play areas, private family dining room, kitchen and wireless connections. The true jewel of the project is The Mandel Rehabilitation Pavilion and 60 bed Post-hospital Care Center featuring Rehab Roadâ„¢, an indoor simulated neighborhood to help relearn the activities of daily living.

Long-Term Care
Montefiore offers long-term care services based on the level of resident needs.
Short-Term Post-Hospital Care
After a surgery, injury or illness, Montefiore's Post-hospital Care Center provides physical, occupational and speech therapy, as well as skilled nursing care and medical monitoring.
Dementia Special Unit
Montefiore's Dementia Special Care Unit is designed for individuals with Alzheimer's and other dementia-related illnesses and is in a home-like secure unit. This unit offers specialized activities, programs and services for its residents.
