Welcome to Heartland of Urbana skilled nursing and rehabilitation center, providing skilled nursing, Alzheimer's care, hospice care and respite stays. Skilled nursing is complemented by physical, occupational and speech therapies. We care for patients with Alzheimer's disease and related disorders within a secure, home-like environment which features an unique lifestyles program, designed to engage each resident to perform day-to-day living activities at his or her optimal level of independence.
Our program staff is specially trained to provide this unique service to our residents and support to their families. In addition, we provide hospice care for those diagnosed with a life limiting illness. A beautifully landscaped courtyard with a fish pond and raised flower beds adds to the serene, home-like environment. The center staff is involved extensively in the community, including an annual chili cook-off with proceeds donated to local organizations.
