Heartland of Perrysburg, Skilled Nursing and Rehabilitation Center, is conveniently located near the I-75 and 475 split in Perrysburg, Ohio.

Today, our center is more like a hospital compared to the nursing homes of years ago. Over 93 percent of our patients are admitted for short-term aggressive therapy as opposed to standard nursing home care. Our medical directors, physiatrist, orthopedic surgeons, and full-time in-house nurse practitioner allow us to care for patients that you would normally expect to find in a hospital setting. Our 2,900 sq. foot therapy gym offers full-time aggressive physical, occupational and speech therapies.

Heartland of Perrysburg is one of the first facilities in Northwest Ohio to offer a dedicated total joint replacement program called Total Joint Camp. Total Joint Camp is an inpatient rehabilitation retreat for patients recovering from joint replacement surgery. This program is designed for patients who have elected to take a proactive approach to return to an active lifestyle as quickly and safely as possible.

In addition to offering a specialized total joint replacement program, our stroke team has had additional education and training in stoke recovery and are highly experienced in rehabilitating stroke patients.

Patients in our rehabilitation program enjoy spacious semi-private rooms separate from our long-term care wing. Amenities include: complimentary cable TV, telephones and an internet cafe. Our restaurant-style menu selection and buffet offers patients a variety of choices.
