Welcome to Heartland of Jackson skilled nursing and rehabilitation center. We are located approximately eight miles south of Jackson, Ohio and six miles north of Oak Hill, Ohio on State Route 93. Our center is on 4 acres of beautifully landscaped grounds, with a breathtaking view of the hills of Southern Ohio. Heartland is recognized in the area for its rehabilitation therapies, both inpatient and outpatient, focused on returning you home.

Our in-house therapists provide physical, occupational and speech therapies in our newly remodeled, physical and occupational therapy gyms. Our Transitional Care Unit for short-term rehab patients is tastefully decorated with flat screen swing arm televisions. Residents and guests also can enjoy a Cafe Lounge. Private and semi-private rooms are offered, based on availability. Each of our staff physicians is located within our county and our medical director is affiliated with one of Southern Ohio's largest hospitals, Holzer Medical Center, as well as the Holzer Clinic of Jackson, Ohio.
