Great Trail Care Center is a leading provider of long-term skilled nursing care and short-term rehabilitation solutions. Our 100 bed facility offers a full continuum of services and care focused around each individual in today’s ever-changing healthcare environment.
Great Trail Care Center has been serving Carroll, Stark and Columbiana Counties since 1986. We are located in a rural setting on the outskirts of a small town. Located next door to Minerva Senior Center, we are also situated near several churches, parks and shopping centers. Our facility is Medicare and Medicaid certified.
