Carlisle Manor is a 48 bed health care facility which has been operated since 1981. Our continuing efforts at excellence have resulted in our recent deficiency free state survey, which ranks us as not only one of the leading facilities in Warren County, but also among the leaders throughout the State of Ohio. Admissions to Carlisle Manor are coordinated by our Director of Admissions, Administrator and Director of Nursing. Upon admission our care plan team carefully designs a rehabilitation plan to meet the individual needs of each resident.

At Carlisle Manor, our residents and their families feel secure knowing that quality medical care is provided 24 hours a day, seven days a week, by caring professionals under the direction of the medical director. Our goal is to provide a plan of individualized care for our residents that will enable them to attain their highest level of independence. We offer a broad range of programs designed specifically to meet the needs of our residents, all requiring various degrees of assistance.

Carlisle Manor relies upon the combined knowledge and skill of a team of professionals to obtain that goal. Our quality staff members are carefully selected, then well trained and managed by experienced supervisors who are outstanding professionals in their fields. Our health care team includes a variety of licensed specialties including physical, occupational and speech therapist, podiatrist, optometrist, audiologist, dentist, and psychologist.

Hospice care is available as well as respite care.

Carlisle Manor accepts Medicare, Medicaid, as well as private pay and insurance. Carlisle Manor is a VA contract provider of extended care. Administrative Services
Our team of dedicated individuals handles resident records in a professional and informative manner. They are able to answer questions not only in medical records but also in Medicare, Medicaid, private insurance, veterans administration and Social Security.

Environmental Services
Our team of maintenance personnel focus their efforts on providing a clean, comfortable home for our residents. Dedicating their time at maintaining the cleanliness of all linens, bedding, and personal clothing of our residents as well as maintaining the cleanliness of the manor itself.

Nursing Services
Our nursing staff is dedicated to the general well being and comfort of each of our residents. Specially trained in geriatrics our nurses are a pleasure to work with.

Nursing care includes:

24 hour a day staff is available to provide a comfortable and safe environment for residents who may be confused

Rehabilitation Services
We believe that the dignity that comes with independence in our residents contributes greatly to their health and well being, and improves the quality of their lives.

Our rehabilitation department's goal is to help our residents in increasing strength, restoring movement and promote healing. Regaining even partial independence contributes to a patient's self esteem. Our in house programs include physical, occupational and speech therapies, and are designed specifically for each resident's needs.

Social Services
Our team offers emotional and supportive assistance for both the family and resident. We assist the residents in meeting their social, spiritual, intellectual, and emotional needs. We also fully explain all local, state, and governmental financial programs and assist in completing all necessary forms. Our team is thorough and strives to meets every possible need that may arise.
